• I understand that any claims up to $250.00 will be processed on receipt of the submitted claim, conditional upon Westfund receiving the original receipt/s or account/s necessary to support the claim within 7 days.

• I understand that claims over $250.00 will be processed within 48 hours upon delivery of the original receipt/s or account/s necessary to support the claim.

• I agree where details of my benefit claim are uncertain. This claim will be processed upon delivery of the original receipt/s or account/s.

EXCEPTIONS

• Specialist Dental claims will be processed upon delivery of the original documentation due to the complexity of processing these claims.

• Medically Recommended Aids require the addition of a letter of recommendation from your Doctor stating that the medical appliance is for the treatment of a medical condition.

• Prescriptions, Vitamins and Vaccination claims will be processed upon delivery of the original documentation due to the complexity of processing these claims.

TERMS OF USE

• I authorise Westfund to contact the relevant service providers to confirm the information provided for each claim if required.

• I certify that I will provide the originals of all receipt/s and account/s to support the following claim/s within 7 days of submission.

• I understand that claims over $250.00 will not be processed until the supporting documents are received by Westfund.
